LiveScan Fingerprinting Form (VOLUNTEER)

Fingerprinting is only required ONCE from each volunteer and is maintained by our Human Resources department. The file remains active throughout the entire time your child attends John Adams Academy. However, if we receive a Department of Justice report that impacts your eligibility to supervise children, we reserve the right to request an updated LiveScan.

We recognize that this process requires both your time and financial support, and we are committed to making it as simple and efficient as possible.

Steps to Complete LiveScan Fingerprinting:

  1. Pick up a pre-printed LiveScan form for each volunteer (and/or driver) from our academy office, or download and print the form from the JAA website. You must take this form with you to the LiveScan agency.
  2. Contact a LiveScan agency of your choice to schedule an appointment.
  3. Bring the following to your appointment:
      - Your completed LiveScan form
      - A valid driver’s license or ID
      - Payment (approximately $50; amount varies by agency)
  4. Return a copy of your completed LiveScan form to the John Adams Academy office. Volunteers are responsible for covering the cost of fingerprinting.

Additional Note: Volunteers who drive scholars on field trips must submit driver forms, driver’s license, and insurance information annually. See the main office or your child’s teacher for more details.
