Driver / Chaperone Document Submission

Any individual, whether staff, parent, guardian, or other that would like a driver/chaperone clearance for a field trip, must complete and submit the items below. They must also keep these items current with JAA as they change or update.

1. DOJ & FBI Live Scan Clearance Form - Download Form
   (Live Scan clearance does not expire once completed for JAA.)
2. Driver Authorization/Chaperone Form - Download Form, Submission via Google Form
   Must be resubmitted yearly. You may add multiple family names and signatures on one form. Please make sure to fill out completely and sign the second page using the insert function or draw function depending on device. 
3. Current Driver’s License
   Front side (only needed if issued a new Driver's License or a new field trip driver to JAA).
4. Current Auto Insurance Declarations Page
   Minimum Requirements: 
   Bodily Injury $100,000/$300,000 
   Property Damage $50,000 
   *Insurance cards and screenshots will not be accepted without a declarations page (see example of documentation).
