Field Trip Procedure & Documents

Field Trip Procedure & Documents

(Board Policy on Field Trips and corresponding process is attached on the right of page for reference.) An online Field Trip Request Form must be submitted at least 60 days prior to date of field trip to be considered for approval. To help facilitate approval process, please follow up with an email to your admin to notify them of form submission. You will be notified when your field trip request has been approved.

Your biggest resource is your  FIELD TRIP COORDINATOR.  

Once Field Trip Request is approved by your administrator, please do the following:

 Use templates below to keep track of donations, lunches, permission slips, chaperone updates, etc.

 Invoices:Email all invoices to Business Tech (ONE month to cut a check)

 Lunches:

Field trips gone a whole day or majority of the day - Lunch needs to be marked YES. To be in compliance we must offer lunches:  When a field trip takes place during your scheduled breakfast or lunch time, you must note that on the permission slip. This ensures that a free, well-balanced meal is available to scholars during field trips, if they occur during a mealtime. Parents can then choose an Academy provided meal or to pack one for their scholar.

Snacks can be brought as a backup by the teacher, but cannot replace a provided meal, and organizing full snack packs for an entire class through parental donations is not allowed. 

Day of Field trip Procedure: Prior to departure, the teacher shall coordinate pick up of lunches from the cafeteria manager. The cafeteria manager and teachers must ensure that the scholars who have Academy provided lunches are offered 2 types of milk, along with their meal for the day. The milk shall be transported in temperature-controlled bags or coolers.

The teacher is responsible for completing a meal service roster during lunch service. This means the teacher is responsible for checking off each scholar’s name when the scholar is handed lunch during lunch service. The meal service roster must be returned to the cafeteria the Lead/Cafeteria Manager can claim lunches for JAA reimbursement. 

Email your cafeteria about your field trip: date and time Lunch Number due date is at 2 weeksFT Coordinator has to give the numbers to Central Kitchen at 2 weeks 

Health/Nurses: Once approved, email nurses/health room. Min 2 weeks needed to prep meds Transportation: Please use private parent vehicles

Field trip coordinators arrange for buses or vans, NOT teachers
